Government Program-Based Eligibility Guidelines
If you are a participant in one or more of the following government assistance programs, you probably qualify for Assurance Wireless phone service in DC:

- National School Lunch (Free Program Only)
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance/Section 8
- Low Income Home Energy Assistance Program (LIHEAP)
- Temporary Assistance for Needy Families (TANF)
If you qualify for Assurance Wireless service under government program-based eligibility guidelines, you are NOW required to send in any supporting documentation at this time. If you qualify under the government program-based eligibility, you do not need to complete the income-based section.
Income-Based Eligibility Guidelines
If you do not qualify under government program eligibility, you may qualify for Assurance Wireless service in Washington DC if your total household income is at or below 135% of the Federal Poverty Guidelines. Find the size of your household below and compare the annual income. If your household income falls below this amount, then you probably qualify for Assurance Wireless service.
